Community Health Worker Program

The Community Health Worker Program has a long history of helping clients navigate the services they need and how to access them.

How can we support you?  

  • We assist you and your children in seeking a primary home/doctor, WIC, a Dentist, Medical /Health insurance, etc.
  • We help you with scheduling you and your children’s appointments.
  • We help with your transportation needs, help with translation, and will advocate on your behalf.
  • We aid you in obtaining a doctor who can address your current health concerns.
  • We can help you find other services in the community.
  • We provide education on health topics and private supportive visits in the comfort of your home.
  • We have Nutrition, Breastfeeding, and Empowerment support groups that you can join.

Courageous Advocates Breaking Boundaries (CABB)

Mission:

The CABB is a safe space where women and mothers empower each other, to amplify their voices, and to learn to be a resource in the community to effect positive change through fostering healthy relationships, skill building opportunities, trainings, and education. The CABB promotes compassion and understanding, first and foremost with ourselves, recognizing that creating change starts within each one of us.

Vision:

The vision of the CABB is to create a community where women and mothers feel nurtured, cared for, and empowered to:

– speak out and not be afraid

– be heard and respected

– develop tools to accomplish their own goals

– be equal participants in decision-making about community resources for women and mothers.

Breastfeeding Support Is Open to All.  This is no cost to attend.
